Roles & Responsibilities

• Conduct clinical screening, comprehensive assessments, diagnosis and treatment planning for patients from birth through elder
• Deliver brief therapy and continuing care, documenting progress and outcomes in a timely, compliant manner
• Lead and coordinate discharge planning for ALF and transitional placements to ensure safe transitions of care
• Collaborate with hospital teams and community partners to connect patients with appropriate services and resources
• Provide client advocacy, assist with complex legal/financial matters like Power of Attorney when relevant
• Maintain accurate records that meet state, federal, accreditation and billing standards
• Participate in multidisciplinary team meetings, contribute to treatment plans, and support staff training efforts
• Complete assigned billing and provider enrollment documentation as requested

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed (or in process) as LCSW, LPC, or LMFT
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical competency (SEARHC) required within 3 months of hire and every three years
• Demonstrated ability in case management, clinical assessment, crisis intervention, and documentation

Education

• Master's Degree in Social Work, Psychology, Counseling or related discipline

Knowledge & Skills

• Discharge planning and care coordination across the continuum
• Interviewing techniques and crisis intervention
• Documentation practices aligned with accreditation and billing requirements
• Understanding of ethical and legal patient rights and compliance
• Case management, community resource coordination, and provider enrollment processes
• Familiarity with tribal health and cultural wellness approaches
• Ability to analyze, evaluate and diagnose situations and recommend effective actions
