Roles & Responsibilities

• Coordinate comprehensive case management for adult and geriatric patients in an acute care setting
• Conduct utilization review and concurrent/continued stay reviews using evidence-based guidelines (InterQual/MCG)
• Lead discharge planning and care coordination to ensure safe, timely transitions
• Document thoroughly and efficiently in Epic to maintain accurate patient records
• Determine medical necessity and support prior authorization and retrospective review processes
• Collaborate closely with interdisciplinary teams including social services and interpretation services
• Manage an active caseload (typical weekday load up to 15 patients; may increase on weekends)
• Ensure compliance with regulatory standards (CMS, NCQA, The Joint Commission, HIPAA)

Qualifications

• Active California state license required (no pending licenses accepted)
• Minimum 2 years of acute care case management experience in California (HARDSTOP)
• Demonstrated experience in Medical-Surgical (MS), MS/Telemetry, ICU and some Emergency Department (ER)
• Proven skills in care coordination, utilization review, discharge planning, InterQual, and Epic (HARDSTOPS)
• Travel/contract experience required (seasoned traveler preferred; first-time travelers typically not accepted)
• Community hospital experience required
• Comfort managing concurrent reviews, prior authorizations, and working with regulatory measures

Education

• Active California RN license required

Knowledge & Skills

• InterQual criteria and utilization review processes
• Epic electronic health record proficiency
• Discharge planning and care coordination workflows
• Prior authorization, pre-cert and retrospective review experience
• Familiarity with CMS, DRG, HEDIS, NCQA, HIPAA, OSHA and The Joint Commission standards
• ICD-10 and CPT coding knowledge (preferred)
• Strong communication and interdisciplinary collaboration skills
• Caseload and time-management skills for fast-paced acute care settings
