Roles & Responsibilities

• Perform thorough assessments and evaluations to create targeted treatment plans
• Deliver evidence-based acute care interventions including gait training and post-op rehab
• Design and teach individualized Home Exercise Programs to accelerate recovery
• Provide clear, compassionate patient and family education to support discharge goals
• Document all patient encounters and progress accurately in Epic
• Coordinate safe discharges and collaborate with the interdisciplinary team
• Use appropriate equipment (gait belts, sliding boards, specialty beds, bed alarms) to ensure patient safety

Qualifications

• Licensed Physical Therapist (LIC REQ) — active license required
• Minimum 1 year of Physical Therapy experience (per job posting)
• Epic charting experience required
• BLS certification preferred
• Experience or comfort working in Acute Care settings; Neurology and Orthopedics experience are a plus

Knowledge & Skills

• Assessment, evaluation, documentation and discharge planning
• Gait training and mobility retraining
• Home Exercise Program development and patient/family education
• Body mechanics and safe patient handling
• Orthopedics and Neurology clinical knowledge
• Familiarity with Epic charting
• Use of bed alarms, gait belts, sliding boards and specialty beds
• Understanding of Commercial, Medicaid and Medicare A billing basics
