Roles & Responsibilities

• Assess and evaluate patients' functional mobility and rehabilitation needs using evidence-based acute care practices
• Design and implement individualized treatment plans, including gait training and home exercise programs, to optimize recovery
• Provide skilled interventions for orthopedic and neurologic populations, including post-op and stroke rehabilitation
• Document thoroughly and accurately in Epic to support patient care continuity and billing requirements
• Educate patients and families on safe mobility, home exercise progression, and discharge planning
• Collaborate with the interdisciplinary team to ensure safe, patient-centered care and appropriate discharge recommendations
• Use assistive devices and safe handling techniques (gait belts, sliding boards, bed alarms) to protect patients and staff

Qualifications

• Licensed Physical Therapist (state license required) — licensure is mandatory
• Minimum 1 year of clinical experience (job lists 1 year of experience)
• Acute care experience strongly preferred; comfortable with orthopedic and neurologic patient populations
• Epic (charting) experience required
• BLS certification preferred
• Familiarity with Medicare A, Medicaid and commercial billing practices preferred
• Ability to work the listed day shift schedule and follow facility dress/uniform guidelines

Knowledge & Skills

• Patient assessment, evaluation and documentation skills
• Gait training, transfers and safe mobility techniques
• Home Exercise Program development and patient/family education
• Orthopedics and neurologic rehabilitation knowledge (post-op care, stroke, spine, hip/knee replacements)
• Experience with equipment: gait belts, sliding boards, hospital/specialty beds, bed alarms
• Proficiency with Epic charting system
• Understanding of Medicare A, Medicaid and commercial billing basics
