Roles & Responsibilities

• Provide skilled physical therapy services to residents at the Eastern Montana Veterans Home
• Assess functional status and develop individualized treatment plans that promote independence
• Deliver therapeutic exercise, gait training, and falls-prevention interventions
• Collaborate closely with nurses, OT, SLP and the interdisciplinary team to optimize resident outcomes
• Document resident progress accurately and timely in the facility medical record
• Educate residents and families on safe mobility strategies and home exercise programs
• Participate in quality improvement initiatives and contribute to a positive care environment

Qualifications

• Current state Physical Therapist license (or eligible to obtain one quickly)
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Comfortable working with geriatric and veteran populations in a long-term/extended care setting
• Excellent communication skills and a collaborative approach to care
• Ability to pass required background checks and facility credentialing

Education

• Doctor of Physical Therapy (DPT) preferred; state licensure required

Knowledge & Skills

• Geriatric rehabilitation principles and falls prevention strategies
• Therapeutic exercise prescription and mobility training
• Gait training, use of assistive devices, and transfer techniques
• Manual therapy and therapeutic modalities knowledge
• Proficient documentation skills and familiarity with electronic medical records
• Strong teamwork, time management, and resident-centered communication
