Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Complete approximately 5 meaningful patient visits each day to support recovery and independence
Document care accurately using the HCHB HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and office manager to coordinate patient care plans
Help geriatric patients with multiple conditions like CHF and COPD improve their quality of life

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Valid driver's license and reliable transportation for community visits

Knowledge & Skills

Strong assessment and treatment planning skills for orthopedic and geriatric populations
Comfortable working independently in home settings
Excellent communication and documentation abilities
Familiarity with HCHB HomeCare HomeBase EMR is a plus
