Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct start-of-care, recertifications, evaluations, and daily visits for home health patients
Develop and implement individualized physical therapy plans for diagnoses including CHF, orthopedic issues, and pneumonia
Collaborate with a clinical manager or educator for field support and guidance
Maintain accurate documentation and coordinate care with the healthcare team

Qualifications

Valid Physical Therapist license in Kentucky (or compact eligibility)
BLS certification from the American Heart Association
Preference for candidates with at least 1 year of experience; new graduates considered on a case-by-case basis
Home health experience is a plus but not required

Knowledge & Skills

Strong clinical skills in treating orthopedic, cardiac, and respiratory conditions
Excellent communication and patient education abilities
Ability to work independently in a home health setting
Familiarity with OASIS documentation and home health regulations
