Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (40 hours/week)

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Coordinate comprehensive care plans for patients in a psychiatric setting
Conduct assessments and facilitate 72-hour holds as needed (5150 certified)
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Provide psychosocial support and crisis intervention
Maintain accurate and timely documentation

Qualifications

Active California LCSW license
Current BLS certification
5150 Certification in California (or willingness to obtain prior to start)
Prior experience with Dignity Health or CommonSpirit facilities preferred
Minimum 2 supervisor references (charge accepted) within the last 12 months or 3 years

Knowledge & Skills

Expertise in psychiatric social work and crisis intervention
Strong care coordination and case management skills
Excellent communication and interpersonal abilities
Familiarity with CA Dignity Health systems and protocols
Ability to work independently and as part of a team
