Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Provide comprehensive home health physical therapy services to patients in their homes
Develop and implement individualized treatment plans to improve mobility and function
Educate patients and families on therapeutic exercises and fall prevention
Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
Document patient progress accurately and timely in accordance with regulatory standards
Adapt therapy techniques to meet the unique needs of each patient

Qualifications

Active California Physical Therapy license (or eligibility to obtain)
Current BLS certification from the American Heart Association
Minimum 1 year of experience in home health or outpatient setting preferred
Strong clinical assessment and communication skills
Ability to travel up to 25% within the service area

Knowledge & Skills

Expertise in home health physical therapy standards and best practices
Proficiency in electronic medical records and documentation
Excellent interpersonal and patient education skills
Time management and independent problem-solving abilities
Knowledge of Medicare home health regulations
