Shift Type: Monday-Friday, 8:30 AM - 5:00 PM (Day); every other weekend required
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate patient care and discharge planning across the continuum to ensure safe, timely transitions
- Assess patient needs and develop individualized plans of care based on evidence-based guidelines
- Determine medical necessity and review utilization to support appropriate resource use
- Manage prior authorizations and appeals/denials processes to keep care moving
- Document thoroughly and accurately in Epic to maintain continuity and compliance
- Collaborate with interdisciplinary teams including social services to address barriers to discharge
- Float between units and facilities as needed to meet patient and operational needs
- Support quality and regulatory initiatives aligned with CMS, NCQA, HEDIS, and The Joint Commission standards
Qualifications
- Current RN license (state licensure required)
- Minimum 3 years of inpatient care management experience (explicit requirement)
- Proficiency with Epic charting system (required)
- BLS certification required; Case Management Certification preferred
- Experience with utilization review, discharge planning, prior authorizations, and medical necessity determination
- Comfort floating between units and facilities and supporting variable schedules
- Reliable vehicle and valid driver’s license required for this assignment
- Able to work every other weekend and complete a 40-hour orientation

- Plan of Care development and Needs Assessment/DME orders
- Epic electronic health record proficiency
- Familiarity with CMS, DRG, HEDIS, HIPAA, NCQA, The Joint Commission, and OSHA
- Strong clinical judgment, communication, and interdisciplinary collaboration skills
