Roles & Responsibilities

• Coordinate patient care and discharge planning across the continuum to ensure safe, timely transitions
• Assess patient needs and develop individualized plans of care based on evidence-based guidelines
• Determine medical necessity and review utilization to support appropriate resource use
• Manage prior authorizations and appeals/denials processes to keep care moving
• Document thoroughly and accurately in Epic to maintain continuity and compliance
• Collaborate with interdisciplinary teams including social services to address barriers to discharge
• Float between units and facilities as needed to meet patient and operational needs
• Support quality and regulatory initiatives aligned with CMS, NCQA, HEDIS, and The Joint Commission standards

Qualifications

• Current RN license (state licensure required)
• Minimum 3 years of inpatient care management experience (explicit requirement)
• Proficiency with Epic charting system (required)
• BLS certification required; Case Management Certification preferred
• Experience with utilization review, discharge planning, prior authorizations, and medical necessity determination
• Comfort floating between units and facilities and supporting variable schedules
• Reliable vehicle and valid driver’s license required for this assignment
• Able to work every other weekend and complete a 40-hour orientation

• Strong clinical judgment, communication, and interdisciplinary collaboration skills
