Roles & Responsibilities

• Perform thorough evaluations of residents' physical abilities, movement limitations, and pain levels to set clear rehabilitation goals
• Develop and implement individualized treatment plans using evidence-based interventions to improve mobility and independence
• Deliver hands-on therapeutic exercise, gait and balance training, and mobility and transfer assistance
• Monitor patient progress, adjust therapy plans as needed, and document all care in accordance with state and federal guidelines
• Collaborate closely with physicians, nursing, occupational and speech therapists, and family members to coordinate comprehensive care
• Educate patients and caregivers on safe mobility techniques, home exercise programs, and safe equipment use
• Supervise PTAs and aides when needed to ensure high-quality, consistent patient care
• Participate actively in interdisciplinary care planning meetings and facility quality initiatives

Qualifications

• Current California license to practice as a Physical Therapist
• Graduate of an accredited Physical Therapy program
• CPR certification (preferred or required depending on facility)
• Experience in a Skilled Nursing Facility or geriatric rehabilitation setting preferred
• Strong documentation skills, including Medicare/Medicaid-compliant charting
• Excellent communication and interpersonal skills for team-based care
• Ability to work independently and supervise PTAs/aides when needed
• Ability to lift, stand, walk, and assist patients with transfers and mobility

Education

• Graduate of an accredited Physical Therapy program (required)
