Roles & Responsibilities

• Evaluate patients to determine physical therapy needs and establish individualized treatment plans
• Develop and implement therapeutic interventions to improve patient function and mobility
• Document patient care accurately and promptly in the EMR in accordance with regulatory and facility standards
• Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient-centered care
• Provide patient and family education to support recovery and safe discharge planning
• Monitor patient progress, adjust treatment plans as needed, and ensure compliance with state and federal PT regulations
• Demonstrate strong clinical reasoning and problem-solving to manage diverse patient populations and complex diagnoses

Qualifications

• Active California Physical Therapy License (in good standing)
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA BLS (Basic Life Support) certification
• 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and general computer skills
• Strong communication, documentation, and problem-solving abilities

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and assessment
• Treatment planning and therapeutic interventions
• Clinical reasoning and decision making
• Accurate documentation and EMR proficiency
• Experience with diverse patient populations and diagnoses
• Strong communication and interdisciplinary collaboration skills
