Roles & Responsibilities

• Provide compassionate, evidence-based in-home physical therapy to adults and seniors
• Assess patient function and develop individualized treatment plans that improve mobility and independence
• Deliver skilled therapeutic interventions and adapt techniques to each home environment
• Document patient progress clearly and timely in the clinical record
• Educate patients and caregivers on safe mobility, home exercise programs, and fall prevention
• Collaborate with nurses, case managers, and the interdisciplinary team to support holistic care
• Manage a flexible caseload and adjust plans as patient needs evolve

Qualifications

• PT or DPT (position lists PT and DPT as required skills)
• All experience levels welcome — any experience, including new graduates
• Comfortable practicing in a home health setting with frequent floating between clients
• Eligibility to practice in the state of Missouri (hold or be able to obtain required state credentials)

Education

• Doctor of Physical Therapy (DPT) or equivalent (PT credentials listed)

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Hands-on therapy techniques and safe patient handling
• Excellent verbal communication and patient education skills
• Time management and organizational skills for visit-based scheduling
• Ability to document thoroughly and accurately
