Roles & Responsibilities

• Perform thorough home health assessments and develop individualized care plans that help patients regain independence
• Provide hands-on treatment and teach effective home exercise programs to improve mobility and function
• Educate patients and families to promote safety, adherence, and long-term outcomes
• Document patient encounters accurately and timely in EPIC and OASIS to support continuity of care
• Manage a productive caseload (may visit up to 6 patients/day; 30 productivity points standard)
• Coordinate with interdisciplinary team members and community resources to optimize patient care
• Travel safely between patient homes using your own transportation and prioritize route efficiency

Qualifications

• Current Physical Therapist license (pending licenses accepted — confirm with hiring manager)
• Minimum 1 year of skilled home health experience required — this role is not open to first-time clinicians
• BCLS (BLS) certification required
• Proficiency with OASIS and EPIC charting required — please flag if you do not have OASIS experience
• Reliable transportation and ability to manage occasional drives up to ~50+ miles
• Facility requires primary series COVID vaccination plus one booster (per special requirements) — confirm details with hiring team

Knowledge & Skills

• Skilled in patient assessment and treatment planning for home health populations
• Strong knowledge of body mechanics and orthopedic post-op care (hips, knees, shoulders, spine)
• Experience with neurologic rehabilitation: post-stroke, TBI, spinal cord injury, Parkinson’s disease, MS
• Ability to design and teach home exercise programs and patient/family education
• Comfortable charting in EPIC and completing OASIS documentation
• Experience across settings such as acute care, outpatient clinic, rehab hospital, and SNF is a plus
