Roles & Responsibilities

Provide compassionate, patient-centered nursing care to assigned residents
Administer medications and treatments accurately and safely
Monitor and document vital signs, observations, and patient changes
Assist residents with activities of daily living and personal care
Communicate clearly with families and the interdisciplinary care team
Follow infection control procedures and maintain a safe environment
Support care planning and contribute to effective discharge transitions

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable administering medications and performing clinical assessments
Background check and immunizations in line with facility policy
Professional, compassionate bedside manner and strong teamwork skills
Previous long-term care or acute care nursing experience preferred

Education

Completed accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

Strong patient assessment and documentation skills
Medication administration and IV/site care familiarity
Electronic medical record (EMR) navigation and charting
Clear verbal and written communication with patients and team
Infection control best practices and safety awareness
Time management and the ability to prioritize tasks under pressure
