Roles & Responsibilities

Provide compassionate direct patient care including basic assessments and vital signs
Administer medications and treatments per facility policy and under appropriate supervision
Document patient status, interventions, and responses accurately and timely in the chart
Assist with activities of daily living and ensure patient comfort and safety
Communicate effectively with the interdisciplinary team and escalate concerns promptly
Support discharge planning and patient education to promote positive outcomes
Participate in unit meetings and quality improvement initiatives to enhance care

Qualifications

Current LPN license in Pennsylvania (required)
Graduate of an accredited practical/vocational nursing program
Current BLS/CPR certification (or willingness to obtain upon hire)
Ability to pass background checks and required health screenings
Strong communication and teamwork skills; a caring attitude toward patients

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

Knowledge of basic nursing assessments and wound care
Medication administration and documentation practices
Familiarity with infection control and safety protocols
Proficient communication with patients, families, and clinical staff
Comfortable using electronic medical records and bedside charting
Time management and the ability to prioritize in a fast-paced setting
