Roles & Responsibilities

Provide compassionate direct patient care and support activities of daily living
Administer medications and treatments safely and accurately
Monitor and document patient vital signs, status changes, and care provided
Collaborate with RNs and interdisciplinary team members to deliver coordinated care
Maintain clear, timely clinical documentation in the facility charting system
Assist with care planning and implement individualized patient interventions
Uphold infection control standards and contribute to a safe care environment

Qualifications

Current, active LPN license (Pennsylvania) — required
Completion of an accredited practical nursing program (diploma or certificate)
Basic Life Support (BLS) certification or willingness to obtain promptly
Comfortable with medication administration, charting, and direct patient care
Strong communication skills and a team-first attitude
Previous long-term care or skilled nursing experience preferred but not required

Education

Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

Medication administration and basic clinical procedures
Vital signs monitoring and recognition of patient status changes
Accurate clinical documentation and charting practices
Infection control principles and safe work practices
Compassionate bedside manner and resident-focused care
Time management and prioritization in a busy facility setting
