Roles & Responsibilities
- Deliver technical nursing care to residents using the nursing process under RN supervision
- Provide skilled nursing care in a Skilled Nursing Facility (SNF), monitoring resident condition and reporting changes
- Assume charge nurse duties when assigned, coordinating care and guiding the nursing team
- Document resident care and treatments accurately using electronic systems and maintain clinical records
- Collaborate with RNs, care team members, and families to implement individualized care plans
- Apply sound clinical judgment to prioritize tasks, plan workload, and ensure resident safety
Qualifications
- Current California LVN license (required)
- Current CPR certification (required)
- Minimum of 1 year of LVN experience
- Graduate of an approved LVN program with an associate degree or equivalent
- Proficient written and verbal communication skills
- Comfortable using computers and electronic documentation systems
Education
- Graduated from an approved LVN program with an associate degree or equivalent
Knowledge & Skills
- Nursing process and clinical judgment
- Skilled Nursing Facility (SNF) practices and resident care
- Charge nurse capabilities and team coordination
- CPR and basic life support knowledge
- Computer proficiency for documentation and charting
- Strong written and verbal communication
