Roles & Responsibilities

• Provide compassionate direct patient care tailored to each resident’s needs
• Administer medications and treatments accurately under scope and supervision
• Monitor and document vital signs and patient status with clear, timely notes
• Assist residents with activities of daily living while promoting independence
• Collaborate with RNs, CNAs and interdisciplinary team members to deliver cohesive care
• Observe and report changes in condition promptly to ensure patient safety
• Maintain infection control practices and a safe, clean environment for residents

Qualifications

• Active LPN (Licensed Practical Nurse) license in Pennsylvania (required)
• Graduate of an accredited practical nursing program or equivalent training
• Current BLS/CPR certification preferred or ability to obtain upon hire
• Comfortable working 8-hour shifts and delivering hands-on resident care
• Clear background check and up-to-date immunizations as required by facility

Education

• Graduation from an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Strong medication administration and documentation skills
• Good assessment skills for monitoring resident status and reporting changes
• Clear, compassionate communication with residents, families and team members
• Basic familiarity with electronic medical records or the ability to learn quickly
• Knowledge of infection control standards and long-term care best practices
• Ability to prioritize tasks in a busy clinical environment
