Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Perform accurate medication administration and timely clinical assessments
• Document resident status and care thoroughly using PCC
• Collaborate with RNs and interdisciplinary team to support care plans
• Assist with admissions, transfers, and routine clinical procedures as needed
• Support resident safety and mobility while promoting independence and dignity
• Communicate clearly with families and care team to ensure continuity of care

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
• Experience or comfort working in Long-Term Care (LTC) environments
• Familiarity with PCC charting and electronic health record workflows
• Ability to float between two floors and adapt to changing unit needs
• Comfort and stamina for 12-hour day shifts (8:00 AM - 8:00 PM)

Education

• LPN diploma or equivalent (Licensed Practical Nurse credential required)

Knowledge & Skills

• Strong clinical assessment and monitoring skills
• Medication administration and documentation proficiency
• Knowledge of LTC best practices and resident-centered care
• Good communication and teamwork skills with nursing staff and leadership
• Ability to prioritize tasks during busy 12-hour shifts
