Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care following facility policies and nursing standards
Administer medications and treatments accurately and document in the medical record
Monitor resident condition and report changes promptly to the care team
Collaborate with RNs, therapists, and support staff to create and follow individualized care plans
Assist with activities of daily living while promoting resident dignity and independence
Participate in facility quality improvement initiatives and safety practices

Qualifications

Current LPN license in Pennsylvania (or compact state eligibility where applicable)
CPR/BLS certification required; additional certifications encouraged
Valid immunizations and ability to pass background and TB screening
Comfortable with medication administration and basic nursing assessments
Strong communication skills and a team-oriented attitude
Previous long-term care or skilled nursing experience preferred but new grads are welcome to apply

Education

LPN diploma or vocational nursing certificate (or equivalent)

Knowledge & Skills

Fundamental nursing assessment and documentation skills
Medication administration and basic IV/SC/IM familiarity as required by facility
Knowledge of infection control practices and patient safety standards
Ability to work in a fast-paced caregiving environment with empathy
Basic computer skills for electronic charting and shift reporting
Team collaboration and effective communication with interdisciplinary staff
