Roles & Responsibilities

Provide compassionate, patient-centered nursing care to rehab residents
Administer medications and treatments safely and accurately
Monitor and document patient condition, vital signs and progress
Communicate effectively with patients, families and interdisciplinary team
Assist with admissions, transfers and discharge planning as needed
Follow infection control standards and facility policies
Support rehabilitation goals by reinforcing care plans and patient education

Qualifications

Active Licensed Practical Nurse (LPN) license — required
Current Basic Life Support (BLS/CPR) certification preferred
Comfortable working overnight (6:00 PM - 6:30 AM) and in a rehab/long-term care setting
Clear background check and drug screen as required by facility
Strong communication skills and ability to work as part of a team

Education

LPN diploma or practical nursing certificate; high school diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Wound care basics and monitoring for changes in patient condition
Accurate vital signs measurement and clinical observation
Electronic medical record (EMR) charting and timely documentation
Patient and family education with a compassionate approach
Time management and prioritization during busy night shifts
