Roles & Responsibilities

• Interpret medication orders and accurately maintain computerized patient medication profiles in the EMR
• Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
• Manage controlled substances in compliance with federal, state and local regulations
• Monitor drug therapy regimens for interactions, allergies and appropriate dosing
• Perform pharmacokinetic dosing per hospital protocols to optimize patient outcomes
• Review culture & sensitivity data and recommend appropriate antibiotic therapy
• Provide clinical consultation and clear medication education to practitioners, patients and families
• Document all clinical activities and interventions thoroughly and professionally
• Participate in quality improvement, medication review activities and formulary stewardship
• Contribute to staff education, orientation and serve as a member of the Code Response Team

Qualifications

• Active Colorado Pharmacist license (required)
• Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
• Epic EMR experience required; Meditech experience or training is a plus
• BLS (AHA) required and ACLS (AHA) required
• Flu vaccine required (no exemptions accepted during flu season)
• Experience with IV/chemotherapy compounding, parenteral nutrition and controlled substances handling preferred
• Strong clinical documentation and medication therapy monitoring experience

Education

• BS Pharmacy - ACPE-accredited School of Pharmacy

Knowledge & Skills

• Epic EMR proficiency and computerized medication profile management
• Medication order interpretation and drug interaction/allergy review
• IV admixture, chemotherapy and parenteral nutrition compounding
• Pharmacokinetic dosing and therapeutic drug monitoring
• Controlled substances handling and record maintenance
• Medication dispensing, formulary management and cost-effective therapy selection
• Clinical consultation, drug information provision and staff education
• Quality improvement, C&S interpretation and participation on code response teams
• Familiarity with Meditech is a plus
