Roles & Responsibilities

• Evaluate patients to determine functional limitations and design individualized outpatient treatment plans
• Deliver evidence-based treatment and rehabilitation to support mobility and independence
• Coordinate care and communicate clearly with patients, families, and the interdisciplinary team
• Document patient progress thoroughly and timely in the medical record
• Travel between clinic locations as needed to provide care at multiple points of service
• Adapt treatment approaches across populations including sports/ortho, pediatric, and neuro rehab

Qualifications

• Current, unencumbered Oregon Physical Therapy license (required)
• Healthcare Provider BLS certification (required)
• Comfortable traveling between multiple points of care during the workday
• Experience in outpatient physical therapy, including patient evaluation and treatment/rehabilitation
• Strong communication skills and ability to collaborate with interdisciplinary teams

Knowledge & Skills

• Patient evaluation and development of effective treatment plans
• Outpatient care and rehabilitation techniques for orthopedics, pediatrics, and neurological cases
• Manual therapy basics, exercise prescription, and functional mobility training
• Experience with balance/dizziness rehab and wheelchair seating considerations
• Thorough documentation and clinical time management across multiple sites
• Compassionate patient education and family/caregiver coaching
