Roles & Responsibilities

• Provide hands-on nursing care to residents in a long term care setting with compassion and professionalism
• Administer medications and treatments accurately and document care thoroughly
• Monitor resident conditions, report changes to the interdisciplinary team, and follow care plans
• Support residents with activities of daily living while promoting independence and dignity
• Follow infection control, safety protocols, and facility policies to ensure a safe environment
• Communicate clearly with families and team members to coordinate excellent resident-centered care

Qualifications

• Current and unencumbered LPN license for the state of New York
• 1-3 years of LPN experience (long term care experience preferred)
• Basic Life Support (BLS) certification or willingness to obtain
• Ability to pass any required background checks and health screenings
• Strong communication skills and a team-focused mindset

Education

• LPN diploma or certificate from an accredited nursing program

Knowledge & Skills

• Medication administration and accurate charting
• Resident assessment and documentation best practices
• Understanding of long term care workflows and care planning
• Infection control, safety standards, and regulatory compliance
• Compassionate bedside manner and time-management skills
