Roles & Responsibilities

• Evaluate and treat outpatient physical therapy patients using evidence-based interventions
• Develop individualized treatment plans and set measurable goals with patients
• Provide clear, timely documentation and maintain accurate medical records
• Communicate effectively with patients, families, and interdisciplinary team members
• Educate patients on home exercise programs and strategies to maximize independence
• Collaborate with clinic staff to optimize patient flow and scheduling

Qualifications

• Minimum 2 years of current outpatient physical therapy experience (required)
• Active New Hampshire physical therapy license (Compact allowed)
• Current American Heart Association BLS certification (required)
• Previous travel experience highly preferred
• Provider enrollment must be completed prior to seeing patients — flexible start date required
• One supervisory signed reference from the past year and complete work history required
• Submission package must include PT skills checklist, license verification, permanent address, and RTO

Knowledge & Skills

• Strong outpatient physical therapy evaluation and treatment skills
• Knowledge of documentation standards and insurance/provider enrollment requirements
• Ability to manage caseloads efficiently while delivering patient-centered care
• Good communication and education skills for patient and family instruction
• Comfort with flexible scheduling and adapting to clinic needs
