Roles & Responsibilities

• Provide compassionate, hands-on care for residents at the rehab center, assisting with activities of daily living
• Monitor and document patient status and vital signs during your shift, communicating changes to the care team
• Support individualized care plans by following directions from nurses and rehabilitation staff
• Maintain a safe, clean environment and follow infection control best practices
• Assist with mobility, transfers, and safe positioning to promote patient independence
• Deliver respectful, patient-centered care that preserves dignity and comfort
• Collaborate with interdisciplinary team members to coordinate timely, effective care
• Report observations and concerns promptly to ensure excellent continuity of care

Qualifications

• Certified Nursing Assistant (CNA) certification (required)
• Ability to work overnight/night-shift per diem assignments (6:30 PM - 6:30 AM)
• Comfort working in a rehabilitation/long-term care setting
• Strong communication skills and the ability to document patient information accurately
• Must meet facility onboarding and background check requirements

Education

• Completion of an approved CNA training program; high school diploma or equivalent preferred

Knowledge & Skills

• Skilled in assisting with ADLs (bathing, dressing, toileting, feeding)
• Knowledge of safe patient handling and transfer techniques
• Basic vital sign measurement and observation skills
• Familiarity with charting and documentation best practices
• Infection control awareness and adherence to protocols
• Teamwork and the ability to follow clinical directions from nursing staff
