Roles & Responsibilities

• Conduct home health physical therapy visits with professionalism and patient-centered focus
• Complete timely, accurate patient documentation after each visit to support continuity of care
• Manage travel and drive time efficiently between patient homes while maintaining safety and punctuality
• Track and meet visit-based productivity expectations (point system) to support weekly visit goals
• Communicate clearly with patients, caregivers, and interdisciplinary team members by phone and in person
• Adhere to infection control practices, including care for COVID+ and PUI patients when required
• Follow agency and client contract requirements, submit requested time-off at time of placement, and maintain schedule commitments

Qualifications

• Current California PTA license in good standing
• Minimum of 1 year professional experience (home health experience preferred)
• BLS (Basic Life Support) certification
• Current TB test (annual), MMR, Varicella, Hepatitis B, Tdap (within 10 years), annual Flu vaccination
• Clean drug screen and recent physical exam as required
• Valid Driver's License, current auto insurance and vehicle registration
• Willingness to travel within assigned counties and provide weekend coverage as needed
• Comfort treating COVID+ and PUI patients when required

Knowledge & Skills

• Home health patient assessment and in-home therapy delivery
• Accurate patient documentation and charting for home visits
• Visit-based productivity tracking and understanding of point-to-hour conversions
• Per-visit billing awareness and ability to document visit types appropriately
• Efficient travel/drive time management and route planning
• Clear telephone and interpersonal communication with patients and referral sources
• Ability to work independently in the community and adapt to varied home environments
