Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across a 60-mile radius
Help patients recover from surgeries, falls, and other conditions with personalized treatment plans
Collaborate with home health and hospice teams to deliver comprehensive care
Document patient progress using HomeCare/HomeBase EMR system
Travel to 5-7 patients daily, bringing expert care directly to those who need it most

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
At least 1 year of previous home health experience
Valid driver's license and auto insurance required
Strong PT-related skills and passion for patient-centered care

Knowledge & Skills

Physical Therapy expertise in home health settings
HomeCare/HomeBase EMR proficiency
Ability to work independently across a wide service area
Excellent communication and interpersonal skills
Compassionate approach to patient care
