Roles & Responsibilities

• Assess patients' functional mobility and develop individualized treatment plans
• Provide therapeutic interventions focused on pain management and mobility training
• Implement fall risk reduction strategies to improve patient safety
• Assist with discharge planning to promote safe transitions home
• Document evaluations, progress notes, discharges and daily patient care thoroughly
• Communicate with physicians, rehab staff, insurers and patients to coordinate care
• Maintain clean and safe treatment areas, including light cleaning and laundry support
• Perform other duties as requested to support patient outcomes and team needs

Qualifications

• Licensed Physical Therapist in Wisconsin or eligible for licensure
• Doctor of Physical Therapy (DPT) preferred; relevant master’s or bachelor’s considered
• Current BLS/CPR certification (or willing to obtain)
• Comfortable with patient assessment, mobility training, pain management and discharge planning
• Strong documentation and communication skills with attention to detail
• Reliable, team-oriented, and willing to help maintain a clean, patient-focused environment

Education

• Doctor of Physical Therapy (DPT) preferred
• Master's or Bachelor's in Physical Therapy or related field considered
