Roles & Responsibilities

• Provide hands-on treatment to patients as directed by the supervising Physical Therapist, helping them regain function and independence
• Document daily treatment notes and weekly progress notes in accordance with PT board and facility policy
• Follow physician treatment plans and record treatment changes per policy
• Communicate patient progress, problems, and plans clearly to supervisors and the interdisciplinary care team
• Teach and train caregivers, families, and nursing staff on maintenance programs to support safe discharge
• Assist with patient care and rehabilitation conferences to coordinate care goals
• Help maintain the treatment area and report equipment issues so the department runs safely and smoothly
• Participate in in-service training programs and support staff education efforts

Qualifications

• Licensed Physical Therapist Assistant in good standing with the state board (license required)
• Proficiency using Casamba and PointClickCare for clinical documentation
• Ability to meet continuing education requirements per state practice
• Excellent communication skills for working with patients, families, and interdisciplinary teams
• Comfort training restorative aides and participating in staff in-service sessions
• Reliable professional approach with strong attention to documentation and compliance

Education

• Licensed Physical Therapist Assistant (state license required)
• Must meet and maintain continuing education requirements per state board

Knowledge & Skills

• Patient care and rehabilitation treatment techniques
• Clinical documentation best practices including daily notes and weekly progress notes
• Caregiver and staff training methods
