Roles & Responsibilities

• Establish collaborative practice agreements with oncologists to optimize chemo and supportive care for inpatient and outpatient patients
• Lead staff development and educational programs for pharmacy, nursing, and medical teams
• Serve as the oncology drug information expert and ensure EHR care plans align with national guidelines
• Round with fellows, NPs, and physicians to recommend medication selection, dosing adjustments, and monitoring
• Deliver patient-centered medication education using the teach-back method for oral oncolytics and IV chemotherapy
• Continuously review and update oncology care plans in the EHR for safety and guideline concordance
• Identify and resolve medication-related problems using clinical data, history, and labs
• Precept pharmacy residents and students, shaping the next generation of oncology clinicians
• Participate actively in Oncology Service Line meetings to improve safety, quality, and outcomes
• Promote rational, cost-effective therapy with attention to cost avoidance and savings

Qualifications

• Licensed Pharmacist (active state licensure required)
• Board Certified Oncology Pharmacist (BPS) preferred/required as listed
• Doctor of Pharmacy (PharmD) degree required
• Completion of PGY-1 residency plus clinical training (minimum 3 years) OR PGY-2 Oncology residency (or equivalent oncology practice experience)
• Minimum of three years of hospital pharmacy experience and three years of oncology practice/training
• Experience precepting residents and students is a plus

Education

• Doctor of Pharmacy (PharmD)
• PGY-1 residency or equivalent clinical pharmacy practice (minimum 3 years) OR PGY-2 residency in Oncology or equivalent oncology practice experience

Knowledge & Skills

• Oncology clinical practice expertise including chemotherapy prescribing and monitoring
• Strong experience with Electronic Health Record review and oncology care plan management
• Medication therapy management and supportive care strategies
• Excellent patient counseling and teach-back education skills
• Ability to collaborate effectively with multidisciplinary teams during rounds
• Medication safety, drug information, and guideline-driven clinical decision-making
• Experience promoting cost-effectiveness and identifying cost avoidance opportunities
