Roles & Responsibilities

• Provide safe, accurate order entry and medication verification to protect patient care
• Check and dispense medications efficiently while following all state and federal regulations
• Deliver clinical patient care across populations from pediatrics to geriatrics
• Rotate into ambulatory clinics (anticoagulation, oncology, or others) as assigned to broaden clinical impact
• Collaborate positively with nurses, providers, pharmacy technicians and the care team to optimize outcomes
• Participate in quality improvement and medication safety initiatives to improve system-wide care
• Maintain clear documentation and communication to support continuity of care

Qualifications

• Current Pennsylvania pharmacist license or eligible for reciprocation within 90 days
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
• Inpatient hospital pharmacy experience required
• Pharmacy residency preferred for ambulatory rotations
• Advanced practice certifications are a plus
• BLS (Healthcare Provider) required for anticoagulation clinic duties; renew every two years
• No retail pharmacy experience will be considered for this role

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) (preferred)

Knowledge & Skills

• Solid knowledge of state and federal pharmacy laws and regulations
• Expertise in medication order entry, verification, checking and dispensing
• Clinical judgment across inpatient and ambulatory settings
• Strong communication and teamwork skills with other health professionals
• Comfort working with diverse patient populations including pediatrics and geriatrics
• Ability to work with pharmacy information systems and maintain accurate documentation
