Roles & Responsibilities

• Provide safe, comprehensive medication management via order entry, verification, medication checking and dispensing
• Participate in patient care activities across populations—from pediatrics to geriatrics—ensuring optimal medication use
• Rotate into ambulatory roles such as anticoagulation clinic or oncology as assigned to support outpatient services
• Collaborate with physicians, nurses, pharmacy techs and interdisciplinary teams to resolve medication-related issues
• Maintain compliance with state and federal pharmacy laws and facility policies
• Document interventions and clinical activities to support quality and safety initiatives

Qualifications

• Current Pennsylvania Pharmacist license OR eligible for Pennsylvania reciprocity within 90 days of hire
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred
• Recent hospital pharmacy experience preferred; ambulatory/residency experience preferred for clinic rotations
• Advanced practice certifications preferred for specialized ambulatory roles
• BLS certification required for anticoagulation clinic duties (and kept current every two years)
• Strong communication skills and ability to work collaboratively with interdisciplinary teams

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) (preferred)

Knowledge & Skills

• Order verification, entry, and medication dispensing expertise
• Inpatient pharmacy workflow and medication safety practices
• Ambulatory care clinic familiarity (anticoagulation, oncology preferred)
• Solid understanding of Pennsylvania and federal pharmacy laws and regulations
• Patient-centered care approach across age ranges
• Clear interdisciplinary communication and teamwork skills
