Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings using sound clinical judgment
• Develop and implement individualized treatment plans to maximize each resident’s functional potential
• Collaborate with nursing, therapy, and interdisciplinary team members for coordinated care
• Document evaluations, treatment plans, and progress notes accurately and in a timely manner per facility and regulatory standards
• Participate in care conferences and contribute meaningfully to care planning
• Engage in quality improvement initiatives to help elevate patient outcomes and facility performance

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical assessment and treatment planning skills
• Excellent communication skills to interact with residents, families, and interdisciplinary teams
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented approach to patient care
• Commitment to compassionate, high-quality rehabilitation services
