Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to maximize functional independence
• Develop and implement individualized treatment plans that help residents reach their highest potential
• Collaborate with an interdisciplinary team to coordinate care and improve outcomes
• Document treatments and progress accurately and timely in compliance with facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives
• Educate residents and families on safe mobility, adaptive strategies, and home programs
• Advocate for resident-centered care and support a positive therapeutic environment

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical reasoning and hands-on patient evaluation skills
• Skilled nursing or post-acute care experience preferred
• Excellent communication and interdisciplinary collaboration abilities
• Dependable, flexible, and team-oriented approach to care
• Commitment to delivering high-quality, compassionate therapy
