Roles & Responsibilities

• Conduct timely psychosocial assessments to understand patient and caregiver needs
• Develop and implement individualized discharge plans and plans of care
• Coordinate care across interdisciplinary teams to ensure smooth transitions
• Lead and participate in care conferences and family meetings to align goals
• Manage case utilization reviews and collaborate on resource allocation
• Arrange DME, community resources, and post-discharge services for safe discharges
• Provide crisis intervention and support for patients with mental health or substance use needs
• Document thoroughly and accurately in Epic to maintain clear patient records
• Support protective services processes, advance directives, and POLST discussions
• Advocate for patients experiencing homelessness and connect them to community supports

Qualifications

• Current California state social work license required (pending license accepted but must be active by start date)
• Minimum 2 years acute medical social work experience with discharge planning and care coordination
• Travel experience required; first-time travelers may be considered after screening
• Proven experience with Epic or similar electronic charting systems
• Comfort working with adult and geriatric inpatient populations in a short-term acute care setting
• Experience supporting patients with substance abuse, homelessness, and mental health crises
• Strong communication skills for family meetings and interdisciplinary collaboration
• Ability to meet departmental scheduling needs, including periodic weekend coverage

Education

• Master of Social Work (MSW) preferred
• Licensed Clinical Social Worker (LCSW) credential preferred
• Must hold CA state social work license upon start

Knowledge & Skills

• Discharge planning and care coordination best practices
• Comprehensive psychosocial assessment and treatment plan development
• Case management and utilization review processes
• Familiarity with medical-surgical and short-term acute care environments
• Knowledge of community resources, DME needs, and transition planning
• Crisis intervention techniques and mental health support
• Understanding of advance directives, POLST, and protective services
• Proficient Epic charting and documentation skills
• Strong organizational skills and ability to manage caseloads up to ~25 patients
