Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a Skilled Nursing Facility setting
• Evaluate patient functional status and develop individualized treatment plans that promote independence
• Deliver therapeutic interventions and document progress in timely, professional notes
• Collaborate with nursing, rehab team members, and families to coordinate comprehensive care
• Educate residents and caregivers on strategies to maximize safety and functional performance
• Participate in team meetings and contribute to discharge planning and transitions of care

Qualifications

• Open to all experience levels, including new grads, as the posting specifies any experience is welcome
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment
• Willingness to float as needed to support resident care across units
• Active professional licensure or ability to obtain licensure per facility and state requirements

Knowledge & Skills

• Skilled in assessment and treatment planning for common SNF diagnoses
• Strong documentation skills and familiarity with clinical charting expectations
• Ability to work collaboratively on interdisciplinary teams
• Effective patient and caregiver education skills
• Adaptability to varying clinical needs and a positive, resident-centered approach
