Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to rehab residents
- Administer medications and treatments safely and accurately
- Monitor and document patient condition, vital signs and progress
- Communicate effectively with patients, families and interdisciplinary team
- Assist with admissions, transfers and discharge planning as needed
- Follow infection control standards and facility policies
- Support rehabilitation goals by reinforcing care plans and patient education
Qualifications
- Active Licensed Practical Nurse (LPN) license — required
- Current Basic Life Support (BLS/CPR) certification preferred
- Comfortable working overnight (6:00 PM - 6:30 AM) and in a rehab/long-term care setting
- Clear background check and drug screen as required by facility
- Strong communication skills and ability to work as part of a team
Education
- LPN diploma or practical nursing certificate; high school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Wound care basics and monitoring for changes in patient condition
- Accurate vital signs measurement and clinical observation
- Electronic medical record (EMR) charting and timely documentation
- Patient and family education with a compassionate approach
- Time management and prioritization during busy night shifts
