Roles & Responsibilities

• Provide compassionate direct patient care and support daily living activities
• Administer medications and treatments per facility policy and scope of practice
• Monitor and document patient vital signs, condition changes, and responses to care
• Collaborate with RNs, therapists and the interdisciplinary team to support patient goals
• Maintain accurate, timely charting in the patient record and communicate concerns
• Follow infection control and safety protocols to protect patients and staff
• Assist with patient education and family support to promote recovery and well-being

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico (required)
• Current Basic Life Support (BLS/CPR) certification (preferred or obtain upon hire)
• Experience in long-term care, rehabilitation, or similar clinical settings preferred
• Comfortable working per diem schedules and adapting to changing assignments
• Ability to pass background check, drug screen and meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing / Vocational Nursing program or equivalent

Knowledge & Skills

• Strong patient assessment and observation skills
• Medication administration and documentation best practices
• Familiarity with electronic health record (EHR) charting
• Clear communication and teamwork across clinical disciplines
• Time management and prioritization in a fast-paced care setting
• Understanding of infection control and safety procedures
