Roles & Responsibilities

• Provide compassionate bedside care and clinical support to assigned residents
• Administer medications and treatments safely and accurately as ordered
• Monitor patient status and communicate changes promptly to the care team
• Document clinical observations, interventions, and patient responses clearly
• Collaborate with nurses, therapists, and physicians to deliver person-centered care
• Assist with admissions, discharges, and transfers while ensuring continuity of care
• Support resident comfort, dignity, and daily living needs with respect and empathy

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing
• Prior LPN experience in a long-term care or skilled nursing setting preferred
• Basic Life Support (BLS) certification preferred or willing to obtain
• Valid immunizations and ability to meet facility health requirements
• Strong communication skills and ability to work well on a team

Education

• Completion of an accredited LPN / Practical Nursing program

Knowledge & Skills

• Solid knowledge of nursing fundamentals and medication administration
• Comfortable with electronic charting and accurate documentation
• Skilled at patient assessment and recognizing changes in condition
• Able to prioritize tasks in a fast-paced clinical environment
• Compassionate approach to resident-centered care and advocacy
• Good teamwork, time management, and problem-solving skills
