Roles & Responsibilities

• Provide compassionate direct patient care and assess patient condition
• Administer medications and treatments according to policy and care plans
• Monitor and document vital signs, intake/output, and patient status
• Collaborate with RNs, physicians, and the care team to deliver safe care
• Maintain accurate, timely charting in the electronic health record
• Assist with patient mobility, activities of daily living, and comfort measures
• Follow infection control practices and facility protocols to keep patients safe

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in New Hampshire
• Current Basic Life Support (BLS) / CPR certification (or willing to obtain)
• Strong clinical judgment and the ability to provide reliable, patient-centered care
• Comfortable working in a fast-paced clinical environment and adapting to varying assignments
• Ability to pass background check and meet facility immunization/health requirements
• Previous LPN clinical experience preferred but motivated new grads are welcome

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

• Medication administration and basic IV/shot experience as applicable
• Patient assessment, wound care, and basic nursing procedures
• Proficient documentation skills and familiarity with EHR systems
• Excellent communication and teamwork skills
• Time management and prioritization for longer day shifts
• Basic knowledge of infection prevention and safety protocols
