Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing setting
• Administer medications and treatments safely and document per facility policy
• Monitor and record vital signs, reporting any changes to the care team promptly
• Support activities of daily living and promote resident independence
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain accurate, timely clinical documentation in the electronic medical record
• Follow infection control, safety protocols, and facility policies at all times
• Contribute to a positive care environment and support resident dignity

Qualifications

• Current Licensed Practical Nurse (LPN/LVN) license in Massachusetts required
• BLS/CPR certification required (or obtain upon hire, depending on facility policy)
• Experience in skilled nursing, rehab, or long-term care preferred
• Strong communication and teamwork skills; reliable and compassionate
• Ability to pass background checks, health screenings, and required onboarding

Education

• Graduate of an accredited Practical Nursing program; LPN/LVN diploma or certificate required
• Registered Nurse (RN) applicants with appropriate credentials may also be considered for related openings

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and restorative nursing skills
• Resident assessment and early recognition of clinical changes
• Familiarity with electronic medical records and charting
• Infection control, safety, and regulatory compliance
• Time management, organization, and effective communication
