Roles & Responsibilities
- Provide safe, compassionate direct patient care including medication administration and treatments
- Perform timely assessments and document changes in patient condition clearly and accurately
- Coordinate with the interdisciplinary team to develop and follow individualized care plans
- Assist with admissions, transfers and discharges to ensure smooth transitions
- Support rehabilitation activities and help residents toward their recovery goals
- Maintain infection control standards and a safe environment for residents and staff
- Communicate effectively with residents, families and team members to promote excellent care
Qualifications
- Active LPN license in Massachusetts (required)
- Current BLS/CPR certification (required or obtain upon hire)
- Prior experience in long-term care, rehabilitation, or skilled nursing preferred
- Up-to-date immunizations and ability to pass background/credential checks
- Strong interpersonal and communication skills; compassionate, resident-focused mindset
Education
- Graduate of an accredited LPN/Vocational Nursing program
Knowledge & Skills
- Medication administration and documentation accuracy
- Basic wound care and restorative nursing skills
- Skilled in resident assessment and care planning
- Familiarity with electronic health records and charting best practices
- Ability to work well in a fast-paced team environment and prioritize tasks
- Good time management and critical thinking under pressure
