Roles & Responsibilities

• Provide compassionate, professional bedside care to residents during night shifts
• Perform thorough patient assessments and communicate changes in condition to the care team
• Administer medications and treatments accurately, following facility policies
• Collaborate with nursing staff, therapists, and physicians to create and follow care plans
• Document patient care and observations clearly and timely in the medical record
• Support residents with activities of daily living while promoting independence and dignity
• Assist with admissions, discharges, and transfer processes as needed

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
• Basic Life Support (BLS) certification required; ACLS or other certifications a plus
• Comfortable working independently on night shift with strong assessment and prioritization skills
• Previous experience in long-term care, rehab, or skilled nursing preferred but not required
• Strong communication skills and a patient-centered approach

Education

• LPN: Completion of an accredited practical nursing program
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care basics and pressure injury prevention
• IV therapy familiarity (if applicable for role filled)
• Skilled in patient assessment, charting, and care planning
• Ability to use electronic medical records and follow facility protocols
• Calm, critical-thinking approach during emergent or high-stress situations
