Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at the skilled nursing facility
• Administer medications and treatments safely and accurately according to orders
• Monitor, assess, and document patient condition changes and communicate them to the care team
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Support residents with activities of daily living while promoting dignity and independence
• Ensure timely, accurate documentation in the medical record and adhere to facility policies and procedures

Qualifications

• Current Licensed Practical Nurse (LPN) license — local candidates preferred; Registered Nurses (RN) may also be considered depending on facility needs
• Basic Life Support (BLS) certification required or obtained upon hire
• Comfort working night shifts in a fast-paced skilled nursing environment
• Strong communication skills and a team-oriented attitude
• Ability to pass required background checks and meet facility onboarding requirements

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and care planning
• Infection control and universal precautions
• Good charting skills and familiarity with electronic health records (EHR) is a plus
• Ability to prioritize and respond effectively in urgent situations
