Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab and nursing setting
- Administer medications and treatments accurately and safely
- Collaborate with interdisciplinary team members to create and follow individualized care plans
- Monitor and document resident status, vital signs, and progress in the medical record
- Support wound care, mobility assistance, and activities of daily living as appropriate
- Communicate clearly with residents and families, offering education and reassurance
- Maintain compliance with facility policies and state nursing regulations
Qualifications
- Active LPN license in Massachusetts (or active RN license if applying as an RN)
- Current BLS/CPR certification preferred or required by facility
- Comfortable working in a long-term care or skilled rehab environment
- Strong communication skills and ability to collaborate with a multidisciplinary team
- Dependable, patient-centered approach with attention to detail
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and assessment
- Skilled in taking and interpreting vital signs
- Familiarity with electronic medical records and charting
- Ability to prioritize tasks and manage time during busy shifts
- Compassionate patient interaction and family communication
