Roles & Responsibilities
- Assess and evaluate patients' functional status, mobility, gait, ROM, and therapeutic needs
- Develop and implement individualized physical therapy treatment plans under physician orders
- Evaluate the home environment and recommend safe, effective interventions
- Instruct patients, families, and caregivers in therapeutic techniques and use of adaptive equipment
- Initiate PT programs and train other personnel or family members on treatment goals and procedures
- Supervise Home Health Aides and coordinate care with interdisciplinary team members
- Prepare and submit clinical and progress documentation, including OASIS-compliant records
- Participate in case conferences, peer consultation, and ongoing competency assessments
- Report patient progress and changes in condition to the supervising physician in a timely manner
- Provide inservice education and consultation to home health staff as needed
Qualifications
- Current California Physical Therapy Assistant license required
- BLS (Basic Life Support) certification required
- Active NPI number required
- Valid driver’s license and proof of automobile insurance required for home visits
- Minimum 2 years of experience in a healthcare setting; home health or community experience preferred
- Experience with OASIS assessments and home health documentation required
- Good oral and written communication skills and strong organizational ability
- Supervisory experience with home health aides is a plus
Knowledge & Skills
- Proficient in patient assessment, mobility and gait evaluation, and ROM testing
- Skilled in therapeutic and rehabilitative treatment planning and initiation
- Experience with OASIS documentation and home health regulatory requirements
- Familiarity with EMR systems; HomeCare HomeBase (HCHB) experience preferred
- Ability to instruct caregivers and provide clear patient/family education
- Competence in clinical progress documentation and reporting to physicians
- Comfort with home environment evaluations and adaptive equipment/prosthetic device training
- Strong organizational skills and ability to participate in case conferences and staff competency activities
