Physical Therapist

Location: Santa Rosa, CA

Start Date: ASAP

Contract Type: Contract

Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Evaluate and assess patients to develop individualized treatment plans that restore mobility and independence
Implement therapeutic interventions to improve strength, balance, coordination, and overall function
Monitor patient progress and adjust treatment plans to achieve the best outcomes
Educate patients and caregivers on home exercises, safety strategies, and injury prevention
Maintain accurate, timely documentation in line with facility and state guidelines
Collaborate with interdisciplinary team members to deliver comprehensive, patient-centered care

Qualifications

Active California Physical Therapist (PT) license
Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy (DPT) or equivalent)
Current BLS/CPR certification
Minimum 1 year of clinical experience preferred; new graduates may be considered depending on the setting
Strong communication and organizational skills with a patient-focused mindset

Education

Doctor of Physical Therapy (DPT) or equivalent from an accredited Physical Therapy program

Knowledge & Skills

Patient assessment and treatment planning
Therapeutic interventions to improve mobility and function
Patient and caregiver education techniques
Clinical documentation and compliance with state/facility standards
Interdisciplinary collaboration and effective communication
Strong organizational skills to manage caseload and treatment goals
