Shift Type: Monday-Friday, Day shift. Start times vary (pick one): 7:00 AM - 3:30 PM OR 7:30 AM - 4:00 PM OR 8:00 AM - 4:30 PM OR 8:30 AM - 5:00 PM. Weekends: required every other weekend (or 4 weekend days in a 4-week schedule).

Roles & Responsibilities

• Coordinate and lead discharge planning to ensure safe, timely transitions from hospital to home or next level of care
• Document thoroughly in Epic, maintaining accurate case notes and utilization review records
• Apply Milliman Guidelines and CMS/HIPAA requirements to utilization review and care decisions
• Collaborate with interdisciplinary teams including social services, pharmacy, PT/OT, and transport to remove barriers to discharge
• Assess patient needs, arrange DME orders, and create individualized plans of care
• Perform care coordination across specialties, advocating for patients and families
• Support department needs by floating within the case management service when required
• Participate in weekend coverage rotation and maintain professionalism during scheduled shifts

Qualifications

• AK RN license in hand required for assignment (no pending license for auto-offer)
• Current BLS required
• ACM (Case Management) certification preferred
• Minimum 1 year of acute care hospital experience required (first-timers accepted)
• Epic EMR experience required (Epic hardstop noted)
• Ability to float within case management scope to cover departmental needs
• Willingness to work scheduled weekends as required; local candidates not accepted (permanent address must be at least 50 miles from facility)

Education

• Graduate of an accredited nursing program (ADN/BSN) or equivalent; active AK RN license required

Knowledge & Skills

• Proficient charting and documentation in Epic
• Knowledge of Milliman Guidelines and utilization review processes
• Strong discharge planning and care coordination skills
• Familiarity with CMS rules and HIPAA regulations
• Comfort working with diverse patient populations from neonates to geriatrics
• Good communication and collaboration skills with interdisciplinary teams
• Basic computer skills, Microsoft Office and Teams preferred
